Please note that we don't participate in any insurance plans. If your plan includes out-of-network benefits, you may be able to submit a claim form for reimbursement. All payments are due at the time of service.
While we are not in-network with insurance providers, we can certainly help you submit a claim if you have out-of-network benefits. The percentage of a claim reimbursed by health insurance for out-of-network providers typically ranges from 50% to 80%, depending on the plan.
If you'd like to verify your out-of-network coverage, just send us the name of your insurance provider along with a copy or picture of your insurance card (front and back). If your plan includes out-of-network benefits, we can go ahead and schedule your appointment and submit a claim for reimbursement on your behalf!
